Bindings. Some prominent psychiatrists have unacceptable ties to the pharmaceutical industry. Many health care agree with it. But no one sees it in their interest to come forward publicly.

Let me summarize it as follows: * Some prominent psychiatrists have unacceptable ties to the pharmaceutical industry. * Many people in the health care system agree with it. * No benefits from the public to come forward with their concerns. * Those who come forward - and who thus represent psychiatry in the public debate - seem uncomfortable with criticism from outside and uøvde in dealing with perceptions across their own. Let me go a little more detail on these points.


Tight bindings.

Firstly, I think we must recognize that today's prominent psychiatrists - prominent in the public or professional sense - that is in a too-close ties to legemiddelindustrien.Det may be that the research or otherwise is governed by this industry, that the "advertising" for the pharmaceutical industry's products for general practitioners and the media, or that they are in a private bond like. their immediate family working in the pharmaceutical industry.

Secondly, I think we must recognize that there are very many people in the health care system that are concerned about these bindings. My understanding is that the inquiries I have received is the tip of an iceberg. A doctor put his concerns in the following way: "Because of some individuals' offending behavior I have to constantly deal with other people's suspicion."


Strikingly rejection.

Thirdly: It is surprising and alarming that none of Norwegian psychiatry public recognizes as legitimate criticism. There's no business in Norway today that honestly can say that everything is as it should være.Slik may not be in psychiatry: None could be bomb-proof that psychiatry related to the pharmaceutical industry have not affected the doctors on a such a way that the use of drugs have been impressed? It is also problematic, not the pharmaceutical industry wants to go into the debate. No one believes that we should not have a pharmaceutical industry. It is important - but it must also recognize its social significance in such a way that it facilitates the unacceptable bonds. And like all other industries should take the criticism seriously.


Defensive and aggressive.

Fourth, there is a remarkable defensive and aggressive response to trace among the representatives of psychiatry that advocates in the debate. I think the moral crisis of psychiatry is a fact. The question now is how the health service, universities, research institutions and pharmaceutical companies choose to handle it. It is no longer a viable way to deny the concern that so many parts. Now is the time to take it seriously. One should ask themselves questions about the ethics addressed in such a way that the public can take for granted the integrity of psychiatry. I am not convinced that that is the case for parts of Norwegian psychiatry today. Can it for example. raised doubts about objectivity with respect to some psychiatrists. their ties to the pharmaceutical industry? I think so. Psychiatry is not served by giving the impression that it does not take hold of this issue.


Another debate.

Let me conclude by pointing out that the scientific debate going on about the extent of use of various drugs in psychiatry in many ways is a different debate. It is of course a very important debate, as long as it is about just under 300 000 people who get discharged antidepressiva.Det is big money and serious humanitarian considerations at stake. But precisely for that reason we must be able to trust those who participate in the debate - and those who are helping to write the kind of drugs. Neither supporters or opponents of the use of drugs in psychiatry is served by suspected partiality.

Psykiatri i moralsk krise

BINDINGER. Enkelte fremstående psykiatere har uakseptable bindinger til legemiddelindustrien. Mange innen helsevesenet er enig i det. Men ingen ser seg tjent med å stå frem offentlig.

Bekymring. Denne høsten har jeg deltatt i en debatt om psykiatriens bindinger til legemiddelindustrien. Jeg sitter igjen med et inntrykk av at norsk psykiatri befinner seg i en moralsk krise.Jeg vet ikke hvor mange henvendelser jeg har fått fra mennesker i helsevesenet som deler min bekymring, men det er svært mange - fra professorer, overleger, psykiatere, leger og fra andre berørte parter, som pasienter og pårørende.


På høy tid.

La meg sitere en henvendelse jeg fikk på e-post: "Bare noen ord fra en stresset lege (endog spesialist i psykiatri) på vakt om at jeg har satt pris på å lese dine avisinnlegg angående psykiatrien og legemiddelindustrien. Det er på høy tid at noen sier ifra, herunder om sentrale fagpersoners åpenbare bindinger og følgende inhabilitet. Så vidt jeg vet, er det ikke minst mange leger som deler ditt syn".Det overrasker meg ikke.De som henvender seg, vil ofte gi meg informasjon og like ofte ønsker de at jeg skal ta opp bestemte forhold som bekymrer dem. Jeg har gjort det til vane å utfordre dem som henvender seg til selv å stå frem, men svaret jeg får er svært ofte følgende: "Jeg tror det er best om du sier dette", eller "Jeg ser meg ikke tjent med selv å stå frem i denne diskusjonen".Jeg har aldri opplevd lignende i en debatt: Jeg opplever nærmest å ha blitt en varslingskanal for mennesker som bekymrer seg for utviklingen i psykiatrien mht. bindinger til legemiddelindustrien. De kontakter meg på telefon, på SMS, på e-post - ja, til og med på butikken.


Selvkritiske røster.

I dag diskuteres varsling i norsk offentlighet, og svært mange virksomheter tar dette meget alvorlig. Men innen psykiatrien kan det synes å være en utbredt oppfatning at man ikke går ut offentlig og tar opp uakseptable forhold. Og den debatten som har funnet sted, er i seg selv et godt eksempel på dette.Det har ikke vært en eneste selvkritisk røst fra psykiatrisk hold, ikke én fagperson har stått frem og tatt kritikken på alvor. Det er meg en gåte at de ikke kan anerkjenne kritiske innspill som legitime. Så hva består den moralske krisen i? La meg sammenfatte den i følgende punkter:* Enkelte fremstående psykiatere har uakseptable bindinger til legemiddelindustrien.* Mange mennesker innen helsevesenet er enig i det. * Ingen ser seg tjent med å stå frem offentlig med sin bekymring.* De som står frem - og som på den måten representerer psykiatrien i det offentlige ordskifte - virker ubekvemme med kritikk utenfra og uøvde i omgang med oppfatninger på tvers av sine egne. La meg gå litt nærmere inn på disse punktene.


Tette bindinger.

For det første: Jeg tror vi må erkjenne at det i dag finnes fremstående psykiatere - fremstående i offentlig eller faglig forstand - som står i en altfor tett binding til legemiddelindustrien.Det kan for eksempel være at de forskningsmessig eller på annen måte er for styrt av denne industrien, at de "reklamerer" for legemiddelindustriens produkter for allmennleger og i mediene, eller at de står i en privat binding som f.eks. at deres nærmeste familie arbeider i legemiddelindustrien.


Toppen av isfjellet.

For det andre: Jeg tror vi må erkjenne at det er veldig mange mennesker innen helsevesenet som bekymrer seg over disse bindingene. Min oppfatning er at de henvendelsene jeg har fått er toppen av et isfjell. En lege formulerte sin bekymring på følgende måte: "På grunn av enkelte individers uakseptable oppførsel må jeg hele tiden forholde meg til andre menneskers mistenksomhet".


Påfallende avvisning.

For det tredje: Det er påfallende og alarmerende at ingen i norsk psykiatri offentlig anerkjenner kritikken som legitim. Det er vel ingen virksomhet i Norge i dag som med hånden på hjertet kan si at alt er som det skal være.Slik kan det heller ikke være innen psykiatrien: Ingen kan vel være bombesikre på at psykiatriens tilknytning til legemiddelindustrien ikke har påvirket legene på en slik måte at bruken av legemidler er blitt tilskyndet? Det er også problematisk at ikke legemiddelindustrien ønsker å gå inn i debatten. Det er ingen som mener at vi ikke bør ha en legemiddelindustri. Den er viktig - men da må den også anerkjenne sin samfunnsmessige betydning på en slik måte at den ikke legger til rette for uakseptable bindinger. Og som all annen industri bør den ta kritikk på alvor.


Defensive og aggressive.

For det fjerde, det er en påfallende defensiv og aggressiv reaksjon å spore hos de representanter for psykiatrien som tar til orde i debatten. Selvtilfredsheten synes å være enorm, og denne arrogansen er et problem.Hvis det er en oppfatning i opinionen - og blant mange fagfolk - at det forekommer uakseptable bindinger, ville norsk psykiatri stått seg på å anerkjenne den bekymringen. Det er ikke et angrep på psykiatrien som sådan, på mange måter bør bekymringen forstås som et forsvar. Vi ønsker simpelthen et helsevesen vi kan stole på.


Fornekter bekymringen.

Jeg tror den moralske krisen i psykiatrien er et faktum. Spørsmålet er nå hvordan helsevesenet, universitetene, forskningsmiljøene og legemiddelindustrien velger å håndtere den. Det er ikke lenger en farbar vei å fornekte den bekymring som så mange deler. Nå er tiden inne for å ta den på alvor. Man bør stille seg spørsmål om etikken ivaretas på en slik måte at opinionen kan ta psykiatriens integritet for gitt. Jeg er ikke overbevist om at det er tilfelle for deler av norsk psykiatri i dag. Kan det f.eks. reises tvil om enkelte psykiateres objektivitet mht. deres bindinger til legemiddelindustrien?Jeg tror det. Psykiatrien er ikke tjent med å gi inntrykk av at den ikke tar tak i denne problemstillingen.


En annen debatt.

La meg avslutningsvis påpeke at den faglige debatten som pågår om omfanget av bruken av forskjellige legemidler innen psykiatri på mange måter er en annen debatt. Det er selvfølgelig en meget viktig debatt, all den tid det for eksempel dreier seg om oppunder 300 000 mennesker som får utskrevet antidepressiva.Det er store penger og alvorlige menneskelige hensyn som står på spill. Men nettopp av den grunn må vi kunne stole på dem som deltar i debatten - og på dem som er med på å skrive ut den slags medikamenter. Hverken tilhengere eller motstandere av bruken av legemidler innen psykiatri er tjent med mistanke om inhabilitet.
